Designated Drivers

On one of my visits to the DMV, I brought home the driver handbook. Reading it, I came across the Designated Driver Program section. I stopped for a moment thinking about it. Do people actually use a designated driver when going to an outing? If so, is the driver picked before leaving for the outing or at the location?
